Well Care Home Health of the Triad, Inc. is seeking a licensed Physical Therapist to assess, plan, and implement patient care in a home health setting. You will admit patients when PT is the primary skilled need and coordinate with families and physicians.

You will communicate clearly, document per policy, and continuously contribute to program effectiveness while maintaining schedules and service quality. A valid NC driver’s license is required.

Qualifikationen

• Graduate of Physical Therapy program from an accredited university.
• Licensure in NC/SC; CPR certification.
• Minimum 1 year experience in home health or PT.
• Knowledge of home health PT procedures and equipment.
• Ability to transfer and guard patients safely.
• Ability to apply manual therapy techniques.
• Professional interaction with public, co-workers, patients, families, and physicians.
• Ability to lift >50 lbs and transfer >100 lbs; frequent reaching/bending.
• Ability to apply clinical principles and develop treatment programs.
• Ability to learn computer technology and supportive services.
• Must have valid NC driver's license and vehicle.
