Job summary

Well Care Home Health of the Southern Triangle, Inc. seeks a licensed Physical Therapist to provide in-home therapy across adolescents and adults in North Carolina and South Carolina. You will assess patients, develop treatment plans, and document progress in accordance with agency guidelines.

The role requires a minimum of 1 year of experience, CPR certification, and the ability to lift and transfer patients while delivering compassionate care in patients’ homes.

Qualifications

• Graduate of Physical Therapy program from an accredited university.
• Licensure in NC/SC; CPR certification.

Responsibilities

• Assess, interpret, plan, implement, and evaluate patients per age/diagnosis.
• Admit patients to the agency when PT is primary skilled need.
• Communicate and document per agency policies and guidelines.
• Contribute to program effectiveness.
• Organize and perform work efficiently.
• Maintain and adjust schedule to enhance agency performance.
• Demonstrate daily commitment to agency values.
• Maintain positive interpersonal relations with all members of the agency.
• Maintain and promote customer satisfaction.
• Demonstrate the mission, vision, and values daily.
