Job summary

Humana Inc, CenterWell Home Health, is seeking a Home Health Physical Therapist to help patients regain strength and independence through in-home care. Reporting to the Clinical Manager, you will plan and administer prescribed PT treatments in patients' homes to achieve the highest possible physical function.

This role requires a current and unrestricted Physical Therapy license and CPR certification, with preference for at least one year of PT experience in a team-based healthcare setting.

Responsibilities

• Plan and administer prescribed physical therapy treatment in patients' homes.
• Assess and document patients' physical strengths to guide treatment goals.
• Develop and implement rehabilitation aligned with physician plans.
• Provide PTAs and Home Health Aides with written care instructions and oversight.
• Document patient observations, interventions, and evaluations promptly.
• Report progress to physician, Clinical Manager, care team, patient, and family.
