Job summary

Humana Inc's CenterWell Home Health is seeking a licensed Physical Therapist to join our patient-facing team. You will deliver therapy in patients' homes, help regain strength and mobility, and report to the Clinical Manager while coordinating with the care team.

This full-time position requires a current PT license and CPR certification, collaboration with healthcare professionals, and driving to patient homes under Humana's safety programs.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsibilities

• Plan and administer prescribed physical therapy treatment in patients' homes to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
