Duke HomeCare & Hospice in Durham, NC, seeks a Physical Therapist to deliver services to patients in home or inpatient settings. You will assess needs, design plans of care, and collaborate with a multidisciplinary team to improve mobility and independence.

The role emphasizes teaching patients and families, documenting progress, and ensuring smooth transitions of care. Licensure in NC is required, with eligibility for compact licensure considered.

Competenze

• Bachelor’s degree in physical therapy (required).
• Current Physical Therapist license; NC license to be obtained within 6 months of start date.
• Master’s or Clinical Doctorate in physical therapy preferred.

Mansioni

• Provide physical therapy services to patients with impairments, functional limitations, and health changes.
• Develop and update plan of care with patient/family and define discharge plan.
• Discharge patient when goals are met and coordinate follow-up as needed.
• Educate patients and caregivers; document progress and outcomes.
• Collaborate with supervisors, peers, and other health professionals to optimize care.
