Job summary

Optimal Home Care is seeking a licensed Physical Therapist to provide in-home patient care across Colorado. You will evaluate, treat, and guide patients to restore function and manage pain while adhering to HIPAA and agency policies.

You will collaborate with physicians, document progress, and educate patients and families. The role offers mileage reimbursement, a company phone, and opportunities for continuing education.

Qualifications

• Active license to practice in Colorado
• Minimum 1 year of Physical Therapist experience
• Excellent written and verbal communication skills
• Strong interpersonal abilities and compassionate patient care

Responsibilities

• Complies with ethical and professional standards of conduct per APTA guidelines.
• Assesses and treats patients to relieve pain and restore function.
• Develops and documents Plan of Care and re-evaluates as needed.
• Coordinates with physicians for Plan of Care and discharges.
• Documents patient progress weekly and educates patients/families on devices.
• Maintains patient safety, HIPAA compliance, and agency policies.
• Attends case conferences and staff development activities.
