Job summary

Optimal Home Care is seeking a dedicated Physical Therapist to provide in-home services to patients in the Colorado area. You will evaluate, treat, and re-evaluate patients, develop Plans of Care, and work with families to maximize quality of life under HIPAA guidelines.

Qualifications

• Must hold an active Physical Therapist license in Colorado.
• Excellent written and verbal communication skills required.
• Strong interpersonal skills and patient-centered approach.

Responsibilities

• Complies with ethical and professional standards set by APTA and HIPAA.
• Evaluates patients using diagnostic and prognostic tests and participates in the Plan of Care.
• Develops and implements a Plan of Care with treatment modalities and frequencies.
• Treats patients to relieve pain or restore function and maintain maximum performance.
• Documents progress weekly and instructs patients and families in devices and home program.
• Plans discharge, prepares summaries and instructions.
• Participates in staff and agency development activities.
