Job summary

Optimal Home Care is seeking a Physical Therapist to provide in-home patient services across Colorado. The therapist will uphold HIPAA and ethical standards while delivering high-quality patient care.

Responsibilities include evaluating patients with physician input, developing a Plan of Care, documenting progress, and guiding families on device usage and home exercises. Collaboration with the care team and discharge planning are essential.

Qualifications

• Active Colorado license required or eligible
• Strong written and verbal communication skills
• Good interpersonal skills and teamwork

Responsibilities

• Complies with APTA standards and HIPAA policies.
• Assists the physician in evaluating patients with tests and plans of care.
• Develops, documents, and reevaluates a Plan of Care with treatment frequencies.
• Treats patients to relieve pain or restore function.
• Reports patient reactions and changes to the physician.
• Records progress notes weekly in the patient record.
• Instructs patients and families in use of devices and home exercise.
• Assists in evaluating home environment and adapting care as needed.
• Plans discharge, prepares summaries, and coordinates with team.
