Summary

We are hiring a Physical Therapist (PT, DPT, RPT) for a Home Health team at Caretenders, a part of LHC Group. Responsibilities

• Assess and evaluate patient care needs related to functional status/mobility, endurance, pain, and other medical conditions.
• Develop treatment plans and perform interventions to improve and enhance patients’ well‑being.
• Track patient progress and report response to treatment and any changes to nursing or physician supervisors.
• Guide and inform patients, families, and health‑care team members regarding care plans, functional limitations, and progress toward goals.
• Assist physicians in developing therapy plans of care.

Education And Experience

• Current Physical Therapy licensure in the state of practice.
• Current CPR certification.
• Valid driver’s license and vehicle insurance; dependable vehicle access.
