Job summary

Home Sweet Home Health Agency is seeking a dedicated Physical Therapist Assistant (PTA) to join our home health team in West Valley, Maricopa County. The PTA will deliver one-on-one therapy in patients’ homes, focusing on rehabilitation, fall prevention, and functional mobility.

You’ll work under a licensed Physical Therapist, coordinating with physicians and RNs to support senior patients and improve independence and quality of life.

Qualifications

• Active Arizona PTA license.
• Current CPR certification (AHA required).
• Minimum 1 year of clinical experience; home health experience preferred.
• Strong interest in home health, rehab, and senior care.
• Excellent communication and organizational skills.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Provide in-home physical therapy services following the established Plan of Care.
• Assist patients with therapeutic exercises, mobility training, and rehab programs.
• Support senior care patients in improving strength, balance, and independence.
• Monitor, document, and report patient progress to the supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs, safety, and fall prevention.
• Collaborate with physicians, RNs, and the interdisciplinary care team.

Your Role as a Home Health PTA (Physical Therapist Assistant)
