Job summary

Home Sweet Home Health Agency is seeking a dedicated Physical Therapist Assistant (PTA) for home health in West Valley, AZ. Per diem, part-time, or full-time schedules offer flexibility to manage patient care in the comfort of patients' homes.

You will deliver one-on-one skilled therapy under a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility to improve independence and quality of life for seniors.

Qualifications

• Active PTA license in Arizona.
• Current CPR certification (AHA required).
• Minimum 1 year of clinical experience (home health preferred).
• Interest in home health, rehab, and senior care.
• Excellent communication and organizational skills.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Provide in-home physical therapy services following the Plan of Care.
• Assist patients with therapeutic exercises, mobility training, and rehab programs.
• Support senior care patients in improving strength, balance, and independence.
• Monitor, document, and report patient progress to the supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs, safety, and fall prevention.
• Collaborate with physicians, RNs, and the interdisciplinary care team.
