Home Sweet Home Health Agency is seeking a dedicated Physical Therapist Assistant (PTA) to join our home health team in Maricopa County. We offer per diem, part-time, and full-time schedules to fit your lifestyle and a mission-driven environment focused on senior care and rehabilitation.

You will deliver one-on-one home-based therapy under a licensed Physical Therapist, helping patients regain function, prevent falls, and improve mobility in their homes.

Qualifications

• Active Arizona PTA license.
• Current CPR certification (AHA required).
• Minimum 1 year of clinical experience (home health preferred).

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:

• Provide in-home physical therapy services following the established Plan of Care


• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs


• Support senior care patients in improving strength, balance, and independence


• Monitor, document, and report patient progress to the supervising Physical Therapist


• Educate patients and caregivers on home exercise programs, safety, and fall prevention


• Collaborate with physicians, RNs, and the interdisciplinary care team

Qualifications:

• Active Arizona Physical Therapist Assistant (PTA) license


• Current CPR certification (AHA required)


• Minimum 1 year of clinical experience (home health experience preferred)


• Strong interest in home health, rehab, and senior care


• Excellent communication and organizational skills
