Position Overview

We are seeking an experienced Physical Therapist (PT) for full-time, part-time, or per diem roles providing home health visits from our York office, located in York, PA. This role involves servicing adult and geriatric clients across York County.

Responsibilities

• Make home visits to clients in designated territories.
• Perform diagnostic tests, including mobility, strength, balance, and safety assessments.
• Develop and implement individualized care plans with therapeutic exercises, gait training, etc.
• Assess and revise care plans, collaborating with nursing staff.
• Educate clients and families on rehabilitative care to promote health and independence.
• Document observations and interventions accurately using electronic tablets.

Qualifications

• Current PA Physical Therapist (PT) license.
• Graduation from an APTA-approved program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Proficiency in electronic medical records; EMR experience preferred.
