Residential Home Health and Residential Hospice is seeking a Home Health Physical Therapist to provide skilled care to patients in their homes in the Gurnee area. The role offers a $20,000 total bonus potential, including a $10,000 first-year success bonus and a $10,000 sign-on bonus paid within the first 18 months.

You will travel to patient homes within the assigned territory and collaborate with an interdisciplinary team while delivering patient-centered care and supporting functional

Qualifications

• Active Physical Therapist license in the state you work in.
• Minimum of 1 year of PT experience.
• Current driver’s license and ability to drive ~20% of your day.

Responsabilités

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence

• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home

• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone

• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work

• One year of experience as a Physical Therapist and the clinical expertise to deliver quality patient care

• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
