Job summary

Chesapeake - Potomac Home Health Agency in Maryland is seeking a licensed Physical Therapist to provide home-based physical therapy services. You will assess, plan, implement, and evaluate patients’ treatment programs in the home setting, coordinating care with families and the health team.

Qualifications

• One year of practice as a Physical Therapist preferred.
• Current Maryland Physical Therapist license required.

Responsibilities

• Conduct a complete physical evaluation to determine appropriate treatment.
• Plan, develop, implement, and evaluate patient PT services.
• Complete a thorough plan of care with measurable goals.
• Provide PT interventions under the MD State Practice Act and educate patients/families.
• Instruct patients/caregivers in home exercise programs and equipment use.
• Attend case conferences for interdisciplinary communication.
• Observe infection control and safety protocols.
• Attend staff meetings and in‑services; perform other duties as assigned.
