Job summary

UnitedHealth Group’s VNA of Maryland is seeking a Physical Therapist for home health in Maryland. The role conducts patient assessments, develops and implements therapy plans, and evaluates progress to improve mobility and function.

A sign-on bonus is available for external candidates and a comprehensive benefits package accompanies the role. Requirements include Maryland PT licensure, valid driver's license and dependable vehicle, and CPR certification (or within 90 days).

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire

Responsibilities

• Evaluates the patient's functional status and PT needs.
• Assists the physician in developing the therapy plan of care.
• Provides services within scope of practice per plan of care with the health care team.
• Observes and reports patient responses to treatment and condition changes.
• Treats patients to relieve pain and restore function.
• Instructs patient and family regarding plan of care, limitations, and progress.
• Visits patients per Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and projected frequency of care.
• Function in diverse home settings regardless of background.
