Job summary
BAYADA Home Health Care is seeking an experienced Physical Therapist to provide home health visits in Calvert County, Maryland. The role offers full-time or part-time options, a $5,000 sign-on bonus for full-time, and a competitive benefits package.
The PT will conduct assessments, develop individualized plans, and collaborate with a multidisciplinary team to support patients staying at home. Responsibilities include home visits, motor and functional testing, wound care, patient and family
Qualifications
- Licensed MD Physical Therapist in good standing.
- Graduation from a program approved by the APTA or equivalent.
- Ability to work independently and manage time effectively.
Responsibilities
- Make home visits to clients in designated geographic territory.
- Perform mobility and functional assessments and balance testing.
- Implement wound care and protective interventions with evidence-based dressings.
- Develop and adjust individualized care plans including therapy and gait training.
- Collaborate with nursing and other disciplines in the care plan.
- Educate clients and families to promote health and independence.
- Accurately document observations and interventions using EMR.
