BAYADA Home Health Care, Inc. is seeking a Home Health Physical Therapist to provide in-home visits across Parkesburg, Cochranville, Atglen and surrounding Chester County areas. A PA PT license and at least 1 year of PT experience are required for full-time or part-time opportunities.

The role involves assessments, customized care plans, and collaboration with a multidisciplinary team. Documentation is performed on a tablet, with BAYADA offering a sign-on bonus for full-time positions.

Competenze

• A current Pennsylvania Physical Therapist license.
• At least 1 year of recent PT experience.
• Graduation from an APTA-approved program (or AMA-affilitated).
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.

Mansioni

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements of joint mobility, strength, balance and safety.
• Develop and implement individualized care plans including manual therapy and gait training.
• Collaborate with nursing and other clinicians as part of the multidisciplinary team.
• Educate clients and families on rehabilitative care and activities for independence.
• Document observations and interventions using the EMR/tablet.
