Job summary

Qualifications

• Active PA PT license required.
• Minimum 1 year PT experience.
• Graduate from PT program approved by APTA or AMA committee.

Responsibilities

• Make home visits to clients in designated territories.
• Perform mobility/strength and balance assessments.
• Develop and revise individualized care plans with manual therapy and gait training.
• Educate clients and families on rehabilitation activities.
• Document observations and interventions in EMR.
• Collaborate with nursing on multidisciplinary care plans.
