Job summary

BAYADA Home Health Care is seeking a Physical Therapist to perform home health visits in Chester County, PA, serving adult and geriatric clients on a per-visit basis. The role offers full-time, part-time, or per diem options and emphasizes independent work within a multidisciplinary team.

Qualifications

Current Pennsylvania PT license.
Graduate from APTA-accredited program as indicated by transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal, organizational, and problem solving skills.
Solid computer skills; EMR experience preferred.

Responsibilities

Make home visits to clients in designated geographic territories.
Perform diagnostic tests and measurements (mobility, gait, strength, balance).
Develop and implement individualized care plans including exercises and gait training.
Educate clients and families in rehabilitative care and activities to promote health and independence.
Accurately document observations and interventions using EMR or touch pad tablet.
