Job summary

BAYADA Home Health Care in Exton, PA is seeking an experienced Physical Therapist for home health visits in Chester County. This role offers full-time, part-time, or per diem options, with a $5,000 sign-on bonus.

A PA PT license and at least one year of clinical experience are required to provide skilled rehabilitative care in clients' homes. You will work as part of a multidisciplinary team, develop individualized treatment plans, educate clients and families, and document progress using EMR.

Qualifications

• Licensed Physical Therapist in Pennsylvania (PA).
• Minimum 1 year of PT experience required.
• Strong assessment, documentation, and communication skills.
• Ability to work independently and in a multi-disciplinary team.

Responsibilities

• Make home visits to clients in designated Chester County areas.
• Perform mobility, gait, strength, and balance assessments.
• Develop and implement individualized care plans with PT treatments.
• Document observations and interventions in EMR.
• Educate clients and families on home care and safety.
