Job summary

UnitedHealth Group's DFW Home Health division is seeking a Physical Therapist to conduct patient assessments, develop and implement treatment plans, and collaborate with a healthcare team to improve mobility and daily function in home settings.

The role offers a full-time schedule with a comprehensive benefits package, and bonus opportunities including a $10,000 bonus for full-time status and relocation assistance to support your assignment in Texas.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patient's functional status and PT needs; assists in developing therapy plan of care.
• Provides PT services within scope of practice and plan of care.
• Observes and reports patient responses to treatment and changes in condition.
• Treats patients to relieve pain, improve function, and maintain maximum function.
• Instructs patient and family about plan of care and progress toward goals.
• Visits patients per Plan of Treatment and submits progress notes.
• Completes evaluations and develops PT plan of care per state guidelines.
• Reports outcomes of evaluation, goals, and cadence of care.
• Function across diverse home environments.
