Job summary

Adoration Health in Belpre, OH seeks a Home Health Physical Therapist to provide patient-centered, 1:1 care in the home setting. You will evaluate and treat patients per physician orders and help them reach independence, coordinating with the treatment team.

The role offers PRN visits, opportunities for growth, and supportive team collaboration. A Bachelor's in PT, 2+ years' experience preferred, valid state license and CPR required.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• New graduates may be considered in select markets.
• At least 2 years of PT experience; hospice/community care preferred.
• Valid license in the state of practice and CPR certification.

Responsibilities

• Perform PT evaluation and treatment per physician orders, including adaptive equipment.
• Establish goals and develop plans to move patients toward independence.
• Maintain communication with treatment team, patient, and family.
• Document services thoroughly for proper billing.
• Supervise PTAs and PT Aides in patient care per state act.

Responsibilities

As a Home Health Physical Therapist , You w ill:

• P erform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
• Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
• Maintain communication among the treatment team, physician, patient, and family members as applicable
• Maintain thorough and timely documentation of services provided to ensure proper billing
• Supervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act

Qualifications

• Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
• New physical therapy graduates may be considered in select markets based on program availability
• At least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
• Valid license form the state of practice
• Possess and maintains current CPR certification
