Qualifications

• Two years of experience in adult physical therapy, preferably home health.
• Current Physical Therapist licensure and CPR certification required.
• Strong physical therapy skills per competency checklist.

Responsibilities

• Perform physical therapy evaluation and treatment based on physician orders.
• Establish goals and develop treatment plans for patients.
• Maintain communication among the treatment team and family members.
• Document services provided for billing purposes.
• Supervise Physical Therapist Assistants and Aides as needed.
