Job summary

Residential Home Health and Hospice is hiring a Physical Therapist (PT) to provide skilled care to patients in their homes in the Sarasota County area. This role includes a $10,000 first-year success bonus based on achieving key milestones.

Qualifications

• Active Physical Therapist license in the state you work (Florida).
• At least 1 year of experience as a Physical Therapist.
• Current driver’s license and ability to drive ~20% of the day to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform comprehensive patient evaluations and develop therapy plans to improve strength, balance and independence.
• Educate patients and caregivers on therapeutic exercises, injury prevention and safe mobility at home.
• Maintain compliant EMR documentation.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

PT Qualifications

• Active Physical Therapist, PT license in the state you work
