Job summary

One Home Health is seeking a compassionate Home Health Physical Therapist to join our growing team serving the Orland Park area. This part-time position offers autonomy, flexible scheduling, and the support needed to help patients recover and thrive at home.

You will conduct in-home evaluations, create care plans, provide interventions to improve mobility, educate patients and caregivers, and maintain compliant documentation.

Qualifications

• Current Illinois Physical Therapist (PT) license in good standing.
• Doctor of Physical Therapy (DPT) degree required.
• Home health experience preferred, but not required.

Responsibilities

• Conduct comprehensive in-home physical therapy evaluations.
• Develop and implement individualized treatment plans.
• Provide skilled therapeutic interventions to improve mobility, function, and quality of life.
• Monitor progress and modify treatment plans as appropriate.
• Educate patients and caregivers on exercises, fall prevention, and home safety.
• Maintain timely clinical documentation in compliance with agency standards.
• Collaborate with physicians, nurses, and interdisciplinary team to coordinate care.
• Adhere to regulatory, clinical, and documentation requirements.
