Job summary

Remedy Home Health Care is hiring a Physical Therapist to provide in-home rehabilitation services across Ventura County. You will focus on fall prevention, mobility and independence for senior patients, conducting assessments, developing personalized plans, and documenting care in the EMR.

Responsibilities

• Perform in-home PT evaluations and reassessments.
• Create and implement personalized rehabilitation plans.
• Improve strength, balance, mobility, and functional outcomes.
• Deliver care in home health, assisted living, and community settings.
• Educate patients and families on fall prevention and safe mobility.

Your Role: Home Health Physical Therapist (PT)

As a Physical Therapist in Home Health, you will provide skilled rehab services tailored to each patient’s needs. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients.

Key Responsibilities:

• Conduct in-home physical therapy evaluations and reassessments
• Develop and implement personalized rehabilitation care plans
