Job summary

Enhabit Home Health & Hospice seeks a dynamic professional to manage relationships with physicians, hospitals and referral networks. You will educate partners on services to drive referral growth and improve patient transitions.

Applicants should have a healthcare-focused degree or equivalent experience, strong communication, and a track record in healthcare business development. In-office attendance is required for collaboration and supervision.

Qualifications

• Bachelor's degree in healthcare administration, marketing, or related field or graduate of nursing, therapy, or social work program.
• Equivalent years of experience considered in lieu of degree.
• Strong knowledge of federal, state, and local laws for Medicare home health operations.

Responsibilities

• Serves as primary relationship manager between Enhabit and referral sources (physicians, hospitals, health systems, senior living, and others).
• Supports referral growth by educating sources on services and program requirements to improve patient outcomes and care transitions.
