CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to support patients in their homes, under the supervision of an Occupational Therapist, delivering personalized rehabilitation plans. You will help patients regain strength, mobility and independence through tailored home programs, collaboration with the care team, and thorough documentation of progress.

This full-time role emphasizes compassionate, in-home care with a 40-hour week and required driving

Formación

• Current OTA licensure and unrestricted license.
• CPR certification is required.
• Valid driver's license with reliable transportation.

Responsabilidades

• Provide therapy services planned, delegated and supervised by the Occupational Therapist.
• Assist in implementing programs to restore and enhance task performance.
• Design/adapt equipment and living environments to improve function.
• Provide treatment to improve strength, coordination, ROM, and independence.
• Document observations and prepare progress reports in patient records.
• Instruct patients and families on home programs and use of adaptive equipment.
• Coordinate care with interdisciplinary team and assist with discharge planning.
• Plan community reintegration activities to support physical reconditioning.
