Job summary

CenterWell Home Health is seeking a dedicated Home Health Occupational Therapist Assistant to join our caring team. You will deliver therapy services under the supervision of an Occupational Therapist to help patients regain strength, mobility and independence at home.

You will assess patient needs, assist in home programs, document progress and coordinate with the interdisciplinary team to support rehabilitation and community reintegration.

Qualifications

• OTA licensure is required.
• CPR certification is required.
• A valid driver's license with auto insurance is required.

Responsibilities

• Provide therapy services planned, delegated and supervised by the O.T. in accordance with the patient's Plan of Treatment.
• Assist in implementing vocational/education programs established by the OT to restore function and promote health.
• Design or adapt equipment and living environments to improve independence and participation.
• Provide therapeutic treatment and instruction per physician orders to improve strength, ROM and function.
• Document patient information, progress notes and reports, and assist with progress reports.
• Instruct patients and family on home programs and use of adaptive equipment.
• Participate in care coordination and discharge planning with the interdisciplinary team.
• Design community reintegration activities to assist physical reconditioning and psychological adjustment.
