Job summary

Qualifications

• Minimum of one year of OTA experience in an acute setting.
• Home care experience is desired.
• Ability to effectively communicate with patients, families, physicians and co-workers.
• Knowledgeable on therapy best practices.
• Ability to make appropriate occupational therapy judgments.
• Ability to identify situations and choose best solutions.
• Detail-oriented and observant.
• Disciplined work ethic with prioritization and timeliness.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Provide skilled OT services in a patient’s home according to the plan of care.
• Observe, record, and report patient changes and progress.
• Assist with functional assessments and interventions to improve status.
• Perform OT procedures and prescribed diagnostics.
• Educate patient and family on treatment procedures and safety.
• Communicate with physicians, therapists, and coworkers; participate in care coordination.
• Educate staff on OT modalities and patient education.
• Maintain documentation and chart audits per agency and Medicare guidelines.
• Drive to patient locations and complete visits within scheduled times.
