Job summary

Freedom Home Health and Hospice Care Services Inc. in Grass Valley, CA seeks a Certified Occupational Therapy Assistant to support the registered OT in implementing care standards and supervising delegated services.

The OTA participates in patient evaluations, teaches daily activities, maintains records, and coordinates with the care team to ensure comprehensive plans of care. A minimum of one year of OTA experience, CA licensure, NBCOT certification, and CPR are required.

Qualifications

• Graduate of an occupational therapy assistant curriculum accredited by the American Occupational Therapy Association.
• Certified with the National Certification Examination of the American Occupational Therapy Association.
• Licensed by the State Board of Healing Arts in accordance with state licensure laws.
• A minimum of one (1) year of clinical experience as a certified occupational therapy assistant.
• Demonstrates good verbal and written communication, and organization skills.
• Possesses and maintains current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

Responsibilities

• Participates in ongoing evaluation of patient’s functional status and home environment as delegated by the OT.
• Teaches new skills or retrains patients in daily activities as per policy.
• Maintains clinical records and reports changes to the OT.
• Submits clinical documentation in accordance with policy.
• Follows treatment programs and documents patient response and progress.
• Maintains communication with other care personnel to promote coordinated care.
• Attends inservices, case conferences, and other meetings for coordinated care.
• Identifies patient and family needs for other home care services and assists with referrals.
• Educates patient’s family/caregiver and other health care personnel about the treatment regime.
• Supervision by the OT at least every 30 days with documented notes.
