Qualifications

• Must be a licensed Occupational Therapist in the state of practice.
• Experience in home health is preferred.
• Ability to assess and plan interventions for adults with functional limitations.

Responsibilities

• Assist patients at home to improve daily living skills and basic motor functions.
• Develop and implement individualized therapy plans.
• Coordinate with physicians, nurses, and family under the plan of care.
