Job summary

Qualifications

• Must hold a degree in Occupational Therapy from an accredited program.
• Current, unrestricted state license as an Occupational Therapist.
• Valid CPR certification.

Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, strength and functional level.
• Administer tests and measurements to determine goals and outcomes.
• Provide written evaluation and plan of care defining OT needs, goals and progress.
• Deliver OT services per physician orders.
• Document timely and maintain plan of care; adjust goals and discharge plans as needed.
• Discharge planning and patient/caregiver education on home exercise program.
• Coordinate with healthcare team and family; serve as clinical resource.
• Supervise OTA and HHA as applicable; complete OASIS where state allows.
