Aveanna Healthcare is seeking a dedicated Occupational Therapist to join our home health team in Florida. You will assess, teach, and deliver therapy interventions directed by the physician, coordinating with a multidisciplinary team to optimize patient well‑being.

You will perform initial and ongoing assessments, develop and document goals, and ensure timely plan of care updates. The role requires state licensure, CPR, and a commitment to high‑quality home care.

Qualifikationen

• Current, unrestricted state license as an Occupational Therapist in the state of practice (Florida).
• Degree in Occupational Therapy from an accredited program.
• Valid CPR certification.

Aufgaben

• Perform initial and ongoing assessments of mobility, coordination, strength, and functional level.
• Create plans of care and document goals, progress, and expected outcomes.
• Deliver occupational therapy services per physician orders in home health.
• Complete timely documentation and adjust goals and discharge plans as needed.
• Coordinate with health care team, families, and caregivers; educate on home exercise plans.
• Assist with OASIS and other state-specific documentation where allowed.
