Job summary

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop retraining programs with measurable objectives.
• Confer with the patient’s physician and health care team to develop or revise the Plan of Care.
• Provide therapeutic treatment and instruction to improve strength, coordination, range of motion and function per physician orders.
• Review and expand retraining programs in line with patient goals and rehab program guidelines.
• Collaborate with vocational team members to align plans with patient needs and capabilities.
• Document patient status, progress and treatment plans accurately and timely.
