Job summary

Elite Home Health is seeking dedicated Occupational Therapists who are passionate about helping patients regain independence, improve daily function, and safely remain in their homes. The OT provides skilled services in patients' homes under physician orders, coordinating with the care team.

You will evaluate functional status, develop treatment plans, supervise OTA when required, and ensure Medicare compliance.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current state OTC license in good standing.
• Minimum 1 year of OT experience; home health or rehab preferred.

Responsibilities

• Patient Evaluation & Care Planning: perform initial and ongoing evaluations and develop usptream plans.
• Skilled Therapy Services: provide OT interventions per orders; guide activities and mobility.
• Patient & Family Education: educate on goals, safety, home adaptations and caregiver coaching.
• Documentation & Compliance: timely notes, plan updates, Medicare/state policy adherence.
