PARS Therapy in Crowley, TX is seeking a dedicated Home Health Occupational Therapist to partner with patients recovering at home. You will empower clients to regain independence by delivering personalized, goal-driven care in the home setting.

As part of our team, you will assess homes, adapt interventions, educate families, and document progress. Texas OT license, liability coverage, and a graduate degree are required; home health experience is preferred.

Qualifications

• Active OT license in Texas (TX) is required.
• Professional liability insurance specific to Texas OT practice.
• Graduate degree or higher in Occupational Therapy.
• Experience in home health or rehabilitation preferred.
• Strong communication and rapport-building with diverse patients.

Responsabilités

• Support clients in developing and maintaining daily living and work skills.
• Assist in implementing individualized treatment plans under supervision.
• Adapt interventions to enhance safety and independence in ADLs and IADLs.
• Educate clients on task breakdown and adaptive strategies.
• Recommend and train on adaptive equipment.
• Assess home environments and suggest modifications.
• Collaborate with families and caregivers; document progress.
• Track progress and maintain up-to-date records.
• Conduct regular follow-ups to support continued progress.
