Job summary

CenterWell Home Health is seeking a dedicated Home Health Occupational Therapist to help patients regain strength, mobility and independence at home through rehabilitation-focused care. You will assess daily living skills, coordinate with physicians and the care team, deliver therapy per physician orders and document progress for Plan of Care and discharge planning.

Emphasis on safe, patient-centered rehabilitation.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient’s physician and other health care team members and participate in development/revision of the Plan of Care.
• Provide therapeutic treatment and instruction to patients to improve/restore strength, coordination, range-of-motion and function.
• Document patient status and progress and submit evaluation and discharge summaries to supervising staff.
