BAYADA Home Health Care, Inc. is seeking a Home Health Occupational Therapist for flexible per diem, part-time, or full-time opportunities in the Gainesville, VA area.

The OT will perform home health visits for adult and geriatric clients in senior living communities, delivering skilled rehabilitative care and collaborating with a multidisciplinary team. Responsibilities include visits across defined territories, comprehensive functional assessments, and developing individualized care plans.

Qualifications

• Current VA Occupational Therapist license.
• Minimum of one year of recent OT experience.
• Graduation from an occupational therapy program accredited by AMA/AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients' functional status and ADL/IADL performance.
• Develop individualized intervention plans to achieve favorable outcomes.
• Assess home environments for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations for better outcomes.
• Educate patients and families in rehabilitative care to promote health and safety.
• Document observations and interventions accurately.
