Job summary

Qualifications

• Active VA Occupational Therapist license.
• Minimum one year of home health experience.
• Ability to work independently in patients' homes.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Evaluate functional status and develop individualized intervention plans.
• Assess home environment for safety and accessibility improvements.
• Identify equipment needs and environmental adaptations.
• Educate patients and families on rehabilitative care and safety.
• Document observations and interventions accurately in EMR.
