Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for per diem or part-time home health visits in the North Tucson area, including Marana, Red Rock, and Casa Grande. You will assess adults and geriatrics, develop intervention plans, and coordinate with a multidisciplinary team to promote safe home environments.

The role requires an active AZ OT license and at least one year of OT experience.

Qualifications

• Active Arizona Occupational Therapist license or eligibility.
• Minimum of one year of OT experience, home health preferred.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status including ADL/IADL.
• Develop individualized intervention plans to improve outcomes.
• Evaluate home environments for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
• Revise care plans and coordinate with nursing in the multidisciplinary team.
• Educate patients and family members on rehabilitative care and activities.
• Document observations and interventions using EMR
