Job summary

BAYADA Home Health Care in Oracle, AZ is seeking an experienced Occupational Therapist (OT) to perform home health visits in the North Tucson area and surrounding territories.

You will evaluate patients’ functional status, develop individualized intervention plans, educate families, and document care using EMR; prior home care experience is preferred, and a current AZ OT license is required.

Qualifications

• Current Arizona Occupational Therapist license.
• Minimum one year of recent work experience as an OT.
• Graduation from an accredited OT program (AMA/AOTA).
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills; problem solving.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status and ADLs/IADLs.
• Assess cognitive, psychological and motor aspects and develop intervention plans.
• Evaluate home environment for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Revise the OT care plan and collaborate with nursing on the multidisciplinary plan.
• Educate patients and family members in rehabilitative care and activities.
• Document observations and interventions using a touch pad tablet.
