Job summary

UnitedHealth Group's Elite Home Health, part of the Optum family, is seeking an Occupational Therapist to assess and address functional status and ADLs in home care. You will collaborate with the care team to ensure individualized plans and patient outcomes.

The role emphasizes evaluating functional capacity, documenting CMS-relevant data, and implementing adaptive strategies across diverse home settings. A valid OT license and transportation are required; CPR is preferred.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current drivers license and vehicle insurance, access to dependable vehicle or public transportation.
• CPR certification preferred or plan to obtain within 90 days of hire.

Responsibilities

• Provides OT services within the scope of practice as defined by state laws and plan of care, coordinating with the health care team.
• Evaluates patients functional status and CMS documentation needs; consults with the physician to develop the therapy plan of care.
• Observes, records, and reports patient responses and condition changes to the nurse supervisor and physician.
• Conducts patient assessments to evaluate functional level using diagnostic and prognostic tests.
• Functions effectively in diverse home environments regardless of age, race, creed, color, sex, disability, or financial condition.
