BAYADA Home Health Care in Concord, New Hampshire, is seeking an experienced Occupational Therapist (OT) for a full-time home health role serving adult and geriatric clients in the Concord area. You will perform home visits, evaluate functional status, plan interventions, and collaborate with a multidisciplinary team to support patients aging in place.

NH license required; minimum one year OT experience; EMR proficiency; strong communication and time management skills.

Competenze

• Current New Hampshire Occupational Therapist license.
• Minimum of one year of OT experience.
• Graduate from AMA/AOTA-accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills; problem solving.
• Solid computer skills; EMR experience preferred.

Mansioni

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients' functional status (ADL/IADL, muscle function, endurance, vision, communication).
• Assess cognitive, psychological and motor performance; develop individualized intervention plans.
• Evaluate home environments for hazards or barriers to independence.
• Identify equipment needs and environmental adaptations.
• Educate patients and families on rehabilitative care and activities.
• Document observations and interventions using EMR on a tablet.
