Job summary

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a full-time role providing home health visits in the Baltimore area, focused in Towson, MD. The OT will deliver rehabilitative care to adult and geriatric patients in their homes as part of a multi-disciplinary team.

The role requires Maryland OT licensure and at least one year of OT experience. Preference given to candidates with home health experience and strong skills in ADLs/IADLs, cognitive and motor

Qualifications

• Maryland OT license current.
• Home health experience preferred but not required.
• Minimum of one year of recent OT experience.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform evaluations of patients’ functional status including ADLs and IADLs.
• Assess cognitive, psychological and motor performance and develop intervention plans.
• Evaluate home environments for hazards to independence.
• Identify equipment needs and environmental adaptations.
• Revise care plans in collaboration with the multidisciplinary team.
• Educate patients and families in rehabilitative care.
• Document observations and interventions accurately in EMR.
