Job summary

Angels Care Home Health is seeking a licensed Occupational Therapist to evaluate patients, develop and update care plans, and coordinate with the care team. The role emphasizes infection control, documentation, and adapting plans based on ongoing assessments.

Two years of OT experience and CPR/Hepatitis profiles are required, with knowledge of durable medical equipment to support home health patients.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• CPR certification is current.
• Hepatitis profile current.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment and identify patient needs and appropriateness of care.
• Document, prepare, and complete a Plan of Care and progress notes.
• Revise the Plan of Care based on evaluation and ongoing assessment.
• Apply infection control and universal precautions in patient care.
• Accept clinical assignments matching education and competence.
• Use effective interpersonal relations to coordinate with the team.
• Assess physical, social, and cultural components of the patient’s occupational environment.
