Job summary

Angels Care Home Health is seeking a licensed Occupational Therapist to perform comprehensive clinical assessments and develop individualized plans of care for patients in their home setting. You will document progress and adjust care plans as needed under physician supervision.

The role emphasizes infection control, collaboration with the care team, and evaluating physical, social, and cultural factors influencing patient occupation.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• Current CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment; identify patient needs and appropriateness of care.
• Document, prepare, and complete a Plan of Care, progress notes, and other clinical records continuously.
• Revise the Plan of Care based on evaluation and ongoing assessment.
• Apply infection control principles and universal precautions in patient care.
• Accept clinical assignments within education and competence to care for patients.
• Use interpersonal relations and communication skills to coordinate with the care team.
• Assess physical, social, and cultural aspects of the patient’s occupational environment.
