Job summary

Angels Care Home Health is seeking a licensed Occupational Therapist to perform clinical assessments, develop and revise Plans of Care, and coordinate with a multidisciplinary team. The role emphasizes infection control, documentation, and patient environment assessment.

Qualifications

• Current state license as an Occupational Therapist.
• Two years of experience as an Occupational Therapist.
• CPR and Hepatitis profile.
• Knowledge of durable medical equipment.

Responsibilities

• Perform clinical assessment and identify patient needs.
• Document, prepare, and complete a Plan of Care and progress notes.
• Revise the Plan of Care based on evaluation and ongoing assessment.
• Apply infection control and universal precautions in patient care.
• Accept clinical assignments aligned with education and competence.
• Use effective interpersonal relations to coordinate with the care team.
• Assess physical, social, and cultural components of the patient’s occupational environment.
