Job summary

Compassus is seeking a Home Health Occupational Therapist to provide quality rehabilitative OT to patients in home health settings. The role includes developing OT plans of care, visiting patients for assessments, implementing care, educating families and team members, evaluating outcomes, and supervising assistants.

Licensing and experience in relevant settings are required or preferred. The position emphasizes adherence to regulations, accurate documentation, and collaboration with clinicians

Qualifications

• Must hold an active Occupational Therapist license in state(s) of employment.
• Experience in home health or rehabilitation settings preferred.
• Strong communication and teamwork skills.

Responsibilities

• Participates in OT plans of care for patients.
• Visits patients for assessment and evaluations; completes relevant assessments.
• Implements plans of care and documents progress.
• Educates patients, families, and team members.
• Evaluates outcomes and adjusts care as needed.
• Supervises OT assistants and aides; assists with discharge planning.
• Ensures care quality and adherence to policies and regulations.

Education

Bachelor of Science in Occupational Therapy

1+ year experience in OT (acute/home health/rehab)
